Work Permit Instructions

The work permit application for Worthington is below these instructions. If your student is interested in seeking out a part time job, the work permit must be completed:

1. Complete the student/applicant information section. This must be signed by a parent/guardian.

2. Have the potential employer fill out and sign the employer section.

3. Have the pediatrician/physician complete and sign the physician section.

4. Submit the fully completed and signed application to the Worthington Education Center at 200 East Wilson Bridge Road in Worthington, along with proof of age for the student. The student will need to sign the work permit, so he/she/they must be present at the Worthington Education Center when forms are submitted.
